A signature procedure

Not simply fat removal. A deliberate reshaping of the torso.

Awake Lipo 360 treats the abdomen, waist, flanks and back as one continuous three-dimensional structure. The goal is not to make each area independently smaller, but to create a cleaner transition from the rib cage through the waist and into the hips.

Dr. Kalsow’s approach is based on finding the patient’s underlying frame, reducing the areas that obscure it, and sculpting the surrounding zones so the waist appears narrower and the torso more balanced from the front, side and back.

Because no two frames are identical, treatment is planned around the individual anatomy rather than a standardized pattern.

Experience that changes the operation

What thousands of Awake Lipo 360 cases teach you.

Pattern recognition

High-volume experience allows subtle differences in torso shape, fat distribution, skin quality and prior surgical change to be recognized before treatment begins.

Revision judgment

Patients also seek Dr. Kalsow after previous liposuction elsewhere. Revision work requires a different strategy: preserving what is good, correcting imbalance and working around scarred tissue.

A consistent aesthetic

The objective is a defined, natural-looking waist and smoother circumferential contour—not simply a maximum-volume number or an identical shape imposed on every patient.

Why patients travel

A destination practice for Awake Lipo 360.

Dr. Kalsow treats patients who come to New York from many U.S. states and from outside the United States. A large portion of the practice has grown through patient referrals and patients who specifically seek his approach to waist and back contouring.

Surgeons from outside the United States have also observed his technique. His practice includes both primary cases and complex revision body contouring.

What defines the approach

  • Complete circumferential planning rather than isolated-area liposuction
  • Strong emphasis on waist narrowing and back contour
  • Strategic fat removal based on the patient’s skeletal frame
  • Small access points placed with scar visibility in mind
  • No drains for routine Awake Lipo 360 in Dr. Kalsow’s technique
  • Personal surgical planning and follow-up with Dr. Kalsow
Procedure overview

What Awake Lipo 360 includes

“Lipo 360” refers to circumferential liposuction of the torso rather than treatment of only the front of the abdomen. Depending on the patient, this may include the upper and lower abdomen, waist, flanks, bra-roll region, lower back and adjacent transition zones.

The exact treatment map is individualized. The areas that need the most aggressive reduction are not always the areas that initially appear largest. Proportion and transition are central to the plan.

Awake surgery

Awake Lipo 360 is performed using local/tumescent anesthesia with the patient awake rather than under general anesthesia. Candidacy depends on the amount of treatment required, medical history, anatomy, comfort considerations and the surgeon’s assessment.

Awake does not mean sensation-free. Patients may feel pressure, movement, vibration and intermittent discomfort. Individual experience varies.

Recovery

What happens after Awake Lipo 360?

Early phase

Swelling, firmness, drainage from access sites, bruising and soreness are expected early in recovery. Compression and postoperative instructions are individualized.

Returning to activity

Many patients are mobile quickly, but the appropriate return to work, exercise and travel depends on the extent of treatment and individual recovery.

Final contour

The waist can look substantially different early, but swelling and tissue remodeling continue for weeks to months. The final result should be judged after healing has progressed.

Frequently Asked Questions

Awake Lipo 360 FAQ

What is Awake Lipo 360?

Awake Lipo 360 is a comprehensive liposuction technique that treats the full circumference of the midsection — front, sides, and back — in one session, performed under local anesthesia while the patient remains awake and comfortable.

What is the difference between Lipo 360 and regular liposuction?

Regular liposuction typically targets one or two specific areas, while Lipo 360 treats the full circumference of the midsection — front, sides, and back — for balanced, all-angle contouring. When performed awake, it also avoids general anesthesia, which can mean a lower risk profile and a smoother recovery.

Am I completely pain-free during awake liposuction?

Local anesthesia numbs the treatment area throughout the procedure, so most patients feel pressure, vibration, or mild discomfort rather than pain. You may also be given oral medication beforehand to help you stay relaxed and comfortable.

Can Awake Lipo 360 create an hourglass shape without fat transfer?

Yes — by removing fat around the waist and flanks while preserving natural curves at the hips and back, Awake Lipo 360 can enhance an hourglass silhouette using liposuction alone. Fat transfer is not required, though some patients choose to combine procedures for additional volume. Dr. Kalsow will discuss the best approach for your goals during your consultation.

Do you use drains?

Drain use depends on the extent of treatment and individual healing factors. Many Awake Lipo 360 patients do not require drains, though Dr. Kalsow will let you know if they're recommended for your specific procedure during your consultation.

Will I have scars?

Awake Lipo 360 is performed through very small incisions, typically just a few millimeters long, placed in inconspicuous areas. These heal into faint marks that fade significantly over time and are rarely noticeable.

Can you correct liposuction performed somewhere else?

Yes, Dr. Kalsow performs revision liposuction to address contour irregularities, asymmetry, or unsatisfactory results from a prior procedure. A consultation will determine the best approach for your specific case.

How much fat can be removed?

The amount of fat that can be safely removed varies by patient, treatment area, and overall health. Dr. Kalsow will evaluate your goals and anatomy during your consultation to determine a safe, effective treatment plan.

What is recovery like after Lipo 360?

Because Awake Lipo 360 avoids general anesthesia, many patients feel more like themselves within a day or two and can resume light activity sooner than with general-anesthesia liposuction. Compression garments and a personalized aftercare plan support healing and final results.

When will I see my final result?

Initial improvement is visible once swelling begins to subside, usually within a few weeks, with final results developing over 3 to 6 months as the body fully heals and skin retracts.

How long do results from Awake Lipo 360 last?

Results are long-lasting as long as a stable weight is maintained, since the fat cells removed during liposuction do not return.

Can I travel to New York for surgery?

Yes, Dr. Kalsow welcomes out-of-town and international patients. His team can help coordinate your consultation, procedure, and recovery timeline to make traveling for surgery as seamless as possible.

How do I know whether I am a candidate?

Good candidates are close to their goal weight, in good general health, and looking to contour stubborn fat that hasn't responded to diet and exercise. The best way to know for certain is a consultation with Dr. Kalsow, who will evaluate your goals and anatomy in person.

Consultation

Your anatomy determines the plan.

A consultation is the point where the procedure becomes specific: your frame, target waist, prior liposuction, skin quality, candidacy for awake surgery and the contour that can realistically be created for you.

Surgical procedures have risks and results vary. A consultation is required to determine candidacy and the appropriate treatment plan.
